幽门螺杆菌与胃食管反流病

随着H.pylori感染率的下降,食管腺癌和胃食管反流病(Gastroesophageal Reflux Disease,以下简称GERD)的发病率却明显上升[1-3],H.pylori和GERD的关系以及GERD中H.pylori是否需要根除治疗一直争论不休。


(一)H.pylori在GERD中的感染率及解读
大多数文献报道H.pylori在GERD中的感染率低于对照组,Raghunath等[4]对严格挑选的20篇有关H.pylori感染率的文章进行系统综述,共4134例患者,其中2418例为对照,发现H.pylori在GERD中的感染率为38.2%,对照为49.5%,(P<0.001)。。近年的研究[5]也支持这一观点。多数研究还表明,随着H.pylori感染率的降低,食管的损伤加重,一般按照无镜下食管炎、反流性食管炎、巴雷特食管、食管癌的顺序排列。Schenk等[[6]报道H.pylori阴性的病人食管炎S-M分级的中位数为3级,而阳性病人中位数为2级( P =0.06), H.pylori阴性的病人巴雷特食管发生率为44.3% ,阳性病人为 20.4%( P=0.006),近期研究也发现这种负相关的存在[7, 8]。Wu等[9]则发现H.pylori感染是唯一和食管炎严重成度成负相关的因素(P=0.011)。进一步的研究发现,在GERD中,cagA阳性的H.pylori也具有此种分布特点[10]。


因此,有人推测H.pylori应该对食管具有保护作用,可以延缓食管疾病的发展,但是在GERD中,胃炎的发生率及程度均轻于对照组,来自中国上海的研究发现胃炎程度和反流性食管炎程度负相关[11]。Jang等[12]对73例反流性食管炎及132无反流性食管炎的对照组进行分析,前者H.pylori感染率低于后者,但是同时其胃体胃窦炎症程度及萎缩程度也低于后者,Tomoyuki等[13]认为H.pylori是通过诱发萎缩性胃炎而抑制反流性食管炎的发生的。Anderson等[14]通过病例对照研究发现H.pylori感染和胃萎缩均与反流性食管炎呈负相关,支持胃萎缩可能是H.pylori抑制反流性食管炎的一个机制,但在没有胃萎缩的患者中H.pylori感染也与反流性食管炎呈负相关,考虑可能还存在其他未知的机制。


(二)根除H.pylori后胃食管反流病的发病率
支持H.pylori对食管具有保护作用的另一有力证据是有资料表明,在消化性溃疡及胃炎病人根除H.pylori后,GERD的发病率比未根除者要高,但是这一证据近年来存有争论。


Labenz等[15]是最早报道对H.pylori阳性的十二指肠溃疡病人(内镜证实无反流性食管炎)成功根除者反流性食管炎的发生率为25.8%,高于根除不成功者的患者;Fallone等[16]]对87例H.pylori阳性的十二指肠溃疡患者行根除治疗,63例根除成功,根除成功者发生GERD的百分比为37%,而不成功者为13%(P=0.04);Hamada等[17]对286例胃溃疡、十二指肠溃疡、胃炎、胃十二指肠溃疡进行了根除治疗,并同时选择了在年龄、疾病相匹配的H.pylori阳性的未作根除治疗的286例病人作为对照,观察3年,应用Kaplan—Meier法分析,根除成功的患者反流性食管炎的发病率为18%,而未进行根除的患者为0.3%(P<0.01),其中胃炎病人根除后GERD发病率为15%,胃溃疡病人为18%,胃十二指肠溃疡病人为13%,十二指肠溃疡病人为4%。


但是,也有许多研究得出相反的结果,认为根除H.pylori与GERD发生无明显关系,有研究者对25例H.pylori阳性的病人进行根除治疗,并和10例健康者对照,发现在H.pylori根除前后,食管酸反流没有明显差异[18],Ott[19]对157例功能性消化不良患者进行根除H.pylori治疗,根除成功者和根除失败者1年内反流性食道炎的发病率并无不同。Vakil等[20]进行了多中心的随机对照研究,242例H.pylori阳性的十二指肠溃疡病人进入试验,新发反流症状在H.pylori根除病人(15%)和H.pylori持续感染病人(22%)之间无明显差异(P=0.47)。对胃溃疡的根除治疗结果也没有发现和GERD发生有关[21]。我国Qian等[22]对近期符合入选标准的随机对照研究进行了Meta分析,认为根除H.pylori与GERD的发生无明显关系。


(三)H.pylori对GERD发病机制的影响
H.pylori可能通过影响LES以及胃内酸度,影响GERD的发生。
下食管括约肌(lower esophageal sphincter,LES)静息压下降及一过性下食管括约肌松弛(TLESR)次数的增多是GERD发生的主要机制之一。H.pylori在胃窦部的感染可以导致血清胃泌素浓度的升高,生理浓度的胃泌素可升高LES的压力,而根除H.pylori后胃泌素浓度降低,但在临床上没有发现H.pylori以及空腹血清胃泌素和LES压力的相关性[23],根除H.pylori后,LES压力的变化也不一致,进一步的研究发现TLESR的频率同H.pylori感染无关[24]。


H.pylori可以影响胃内酸度早已得到证实。H.pylori的感染可以导致胃炎的发生, CagA阳性的H.pylori感染的胃炎更加严重。H.pylori对胃内酸度的影响主要取决于H.pylori的分布,胃窦炎时胃酸分泌增加,发生十二指肠溃疡的风险增加[25],胃体炎时胃酸分泌可以减少,酸泌量同胃体炎的严重程度呈负相关,此外,CagA阳性H.pylori可以通过破坏胃腺加速胃体的多点萎缩,从而使胃的泌酸量减少,降低胃内酸度。在对胃酸过低的病人根除H.pylori后,胃酸分泌可以达到正常或者几乎正常的水平,从而促进GERD的发生。西方人群中H.pylori感染引起的胃炎主要以胃窦炎症为主,因此在这类人群根除H.pylori后的随访中发现烧心、反酸症状得到明显缓解[26]。因此,根除H.pylori后GERD症状可能减轻、加重或不变。


(四)H.pylori对GERD治疗的影响
1. H.pylori和GERD的治疗效果
理论上讲,如果H.pylori对食管具有保护作用,那么具有H.pylori感染的GERD在同等治疗条件下应该具有更好的治疗效果,这在某些研究中得到证实,而在另外一些报道中却没有观察到相同的结果。


Holtmann等[27]进行了多中心双盲研究,对846例经内镜证实S-MⅡ、Ⅲ级反流性食管炎病人应用潘妥拉唑40mg qd进行治疗,结果H.pylori阳性的病人,4周治愈率86.6%,而阴性的为76.3%( P = 0.0005 ),8周为96.4%,阴性的为91.8%(P < 0.004),具有明显差异,而且,症状的缓解率H.pylori阳性的病人也好于阴性的病人(P < 0.05 ), Calleja等[28]应用潘妥拉唑40mg qd 对227例内镜下诊断为反流性食管炎的病人进行治疗共24周,发现在第8周时, H.pylori阳性患者其烧心症状的缓解率要高于H.pylori阴性患者;而在第24周时,H.pylori阴性组的复发率(25.9%)明显高于阳性组(10.2%, p < 0.020)。Meneghelli等[29]对256例反流性食管炎应用潘妥拉唑及雷尼替丁进行双盲治疗研究,结果在潘妥拉唑组,H.pylori阳性患者愈合率为76%,而阴性患者为45%;这种影响可能和在H.pylori阳性时,质子泵抑制剂药物具有更高的抑酸效果有关。


相反,也有H.pylori阳性或阴性并不影响GERD疗效的报道。Peters等[30]把具有胃食管反流的巴雷特食管病随机分为奥美拉唑40mg bid及雷尼替丁150mg bid治疗组,发现H.pylori对症状及食管酸暴露并无影响,对奥美拉唑及雷尼替丁在减少酸反流及改善症状方面也无影响,抑酸药物的剂量无需根据H.pylori存在与否而调整,但是实际上该实验所用奥美拉唑剂量远远超过标准剂量,有可能掩盖H.pylori的作用。Tefera等选择了25例H.pylori阳性的反流性食管炎病人进行根除治疗,发现根除治疗前后24小时内pH<4的时间百分率并无明显变化(P=0.46),在9例根除的病人中,酸暴露时间增加,而14例病人酸暴露时间减少,反流症状的积分无明显的变化,酸暴露时间的改变和症状改善无关系,在H.pylori根除后12周及根除H.pylori后15周也无一致的变化[31],但是,该实验样本过少。Vakil等[32]也认为H.pylori和GERD的治疗效果无关,但是,其H.pylori感染的诊断依赖于血清学,结果可信性受到影响。Saad等[33]对包括美国、欧洲、南美、远东等不同人群的随机对照研究进行Meta分析,并未发现H.pylori对GERD的保护作用,他们认为H.pylori感染并不影响GERD症状的改善或者恶化。


之所以有不同的实验结果,可能与病例的选择,药物的用量有关系;也可能表明H.pylori在GERD中并不是决定性因素。


2.H.pylori在长期抑酸治疗时是否导致胃萎缩
GERD需要长期应用质子泵抑制剂,在H.pylori阳性患者可能否导致胃体腺的萎缩。
Schenk等[34]进行了随机对照研究,应用奥美拉唑40mg qd对反流性食管炎进行为期12个月的治疗,并对 H.pylori阳性的患者,加用奥美拉唑20mg、阿莫西林1000mg、克拉霉素500mg bid根除或给与安慰剂共一周,发现H.pylori持续阳性的病人(24例)在治疗期间,胃体部的活动性炎症进展(P=0.032)而胃窦部的活动性炎症好转(P=0.002),而根除成功的患者(33例),胃体部及胃窦部的慢性及活动性炎症均好转(p≤0.0001),和H.pylori持续阳性的病人比较,活动性及慢性炎症的好转具有明显差异(P=0.001),至于萎缩评分,在随访一年内,H.pylori根除组和持续阳性组无明显差异,在H.pylori阴性的病人中未发现变化。提示根除H.pylori,可以阻止强抑酸药物相关性胃体炎的进展,而根除H.pylori是否可以阻止萎缩性胃炎的进展,需要更长的随访。Moayyedi等[35]进行了一项随机双盲前瞻性研究,将44例H.pylori阳性的GERD患者随机分为根除H.pylori组和单纯奥美拉唑组,另有一组H.pylori阴性的GERD作为对照,结果和对照组比较,胃窦部的炎症在单用奥美拉唑组(p =0.008)及根除组(p =0.035)均明显减轻,但在单用奥美拉唑组,胃体部的炎症恶化(p =0.0156),而在根除组,胃体部的炎症好转。在单用奥美拉唑组,胃窦部的炎症为主变为胃体部的炎症为主(p =0.0078),而且11例单用奥美拉唑组中,有5例发展到了轻度的胃萎缩,而在已经根除了8例的病人中,无一例发展成为萎缩,两组之间具有明显差异(p =0.02),提示在H.pylori阳性的GERD中,长期抑酸治疗将导致胃窦部为主的炎症变为胃体部为主的炎症,并伴有胃体部的萎缩的发展,而提前根除H.pylori可以阻止这一转变的发生。


3.H.pylori和质子泵抑制剂的抑酸效果
Verdú 等首先验证了H.pylori可以提高奥美拉唑的抑酸效果, Gillen等[36]的研究不仅证实了H.pylori可以影响奥美拉唑的抑酸效果,并进一步观察到H.pylori阳性者基础泌酸量和最大泌酸量明显的低于H.pylori阴性者。H.pylori对兰索拉唑及潘妥拉唑抑酸效果的影响同样得到证实,关于H.pylori可以提高质子泵抑制剂抑酸效果的机制现在并不清楚。有人提出是氨影响了在奥美拉唑治疗期间的胃内pH[[37],也有人指出由于抑酸治疗,导致了H.pylori在胃内的再分布,伴随着胃窦部炎症的好转而胃体部炎症的恶化,从而泌酸减少,结果是抑酸药物效果的提高。Beil等[38]经过动物实验证实H.pylori可以增强奥美拉唑对壁细胞及H+/K+ATP抑制作用。


4.H.pylori和GERD的复发
GERD症状控制之后容易复发, H.pylori和GERD的复发有无关系,目前尚存争论。有人[39]追踪230例顽固性反流性食管炎病人,在1490个治疗年中,有158例复发,H.pylori阳性或阴性之间并无复发率的不同[39]。但是也有报道,H.pylori阳性GERD者比H.pylori阴性者复发要早, Schwizer等[40]作了随机双盲试验,70例GERD患者在最初10天应用兰索拉唑30mg bid治疗,随后兰索拉唑30mg qd治疗8周,对H.pylori阳性的病人,在最初10天给与克拉霉素500mg和阿莫西林1000mg或安慰剂治疗,结果H.pylori阳性的病人54天复发,而根除成功 者100天复发(P=0.046),H.pylori阴性患者110天复发,在排除食管炎的严重程度影响后,H.pylori阳性患者复发要早于根除成功者(P=0.086)和对照组(P=0.001),因此认为应该对H.pylori阳性的GERD患者进行根除治疗。不过,有报道[41]称根除H.pylori后导致GERD复发率提高。


总之, H.pylori和GERD的关系研究仍存有矛盾。Maastricht IV/ Florence马斯特里赫特IV-佛罗伦萨共识报告[42] 指出:一般而言,H.pylori感染对GERD症状的严重性、症状复发和治疗效果无影响,根除H.pylori不会加重原本存在的GERD,不会影响治疗效果。在GERD中,H.pylori尽管可以提高PPI制剂的抑酸效果,但是这种益处是以加重胃炎的方式来实现的,而且对H.pylori阳性的GERD长期应用PPI制剂可以导致胃黏膜萎缩,而根除治疗可以避免此类事件的发生。因此,我国的共识推荐对需要长期PPI抑酸治疗的H.pylori阳性GERD患者,需要根除治疗[43],这一共识和美国2013<<gerd的诊断及治疗指南>>[44]有所不同,该指南并不推荐在GERD中常规筛检H.pylori,根除H.pylori不作为GERD的抑酸治疗的常规要求。这一条在该指南中虽然属于强烈推荐,但是证据级别较低,更多地可能是配合FDA的结论。
